Using document templates
The Templates tab of the case record provides a series of document templates you can use when you need to create a printed document based on some aspect of the case. The templates extract relevant information from the case or client record to populate the document, for example, client name, UR number, address or date of birth.
| Since these are generic documents common to all local services you need to print them on your agency letterhead. |
The templates you can use are:
Attendance Confirmation Letter – send a letter to your client confirming their expected attendance at an appointment.
Copy of Correspondence – cover letter to accompany copies of all correspondence received on the client’s behalf (usually by a financial counsellor).
Closure for Client – notice of case closure from a financial counsellor after client disengages without notice.
Consent to Share Information – to record freely given informed consumer consent to share their information with a specific agency/ies for a specific purpose/s.
Discharge Summary – a discharge checklist for the client listing goals, plans and resources to help them maintain their progress.
DNA Letter – letter to client after they failed to attend an appointment without notice of cancellation.
Letter of Support to Corrections – letter of support to Corrections from a therapeutic counsellor
Referral Authorisation – authorisation for contact from another local service where client is changing agencies.
Case Activity Report – list of all activities (all statuses) in a case including case notes.

Generating a template document
To generate a template document:
1 Open the case record from which you want to generate a template document.
2 Click the Templates tab.
The templates list displays.
3 In the
Action column of the list click

for the template you want to use.
The Please select party… popup displays.
4 In the
Party field

select the client for whom you want to generate the template.
5 Click
Gambler’s Help Connect generates a word document based on the template and displays it in the Documents previously generated list.
6 In the Documents previously generated section, click the link to the document in the File name column.
The document is downloaded to your PC.

7 You can now open the document for further editing or printing.
